Centralmed ensures the completion of Annex D of the Single Report – the annual activity report model for Occupational Safety and Health services.
The completion of this template is prepared according to the information obtained through medical examinations, safety inspections of workplaces, training hours in the field of occupational safety and health, and other information provided by the employer.
The submission deadline for the SINGLE REPORT is, as usual, from March 15 to April 15, with the auxiliary elements for its completion available on the Single Report and ACT (Authority for Working Conditions) websites.
ANNEX D refers to the annual report of the activities of the safety and health service.
Employers covered by the Labor Code and specific legislation arising from it.
This responsibility lies with the employer.
To update/validate the data necessary for the submission of Annex D of the Single Report, this same update must be carried out through the client portal. Access to the client portal can always be requested by the client from Centralmed.
To delegate to CENTRALMED the submission of ANNEX D directly on the Single Report website, access with the password that was sent to you. The steps on the Single Report platform are as follows: click on access > RU delegations > create delegation > CENTRALMED NIF (503881597) and choose the option « SST > submission » (a new delegation is required even if it has been done in previous years).
You should also validate the business structure, verifying all company data and the respective establishments. If this operation is not carried out, CENTRALMED will not be able to prepare and submit ANNEX D of the SINGLE REPORT.
This procedure triggers an automatic sending of an informative email to the email relatoriounico@centralmed.pt, of the delegation carried out.
This procedure, if chosen, must be done every year after the official opening of the Single Report platform.
